Login to your Disney Plus account at disneyplus.com
Select the profile that manages the subscription
In the top right hand corner click your name/icon and from the dropdown menu click "Account"
Then click on your current subscription from the "Subscriptions" list
From your "Subscription Details" page simply scroll to the bottom and click "Cancel Subscription".
Disney Plus then may offer you alternative plans continue clicking through this to finish the cancellation process.
